To become an UT FCU Online Banking member please print and complete the Online
Banking Application. Once you have read and completed this form you may take it to one
of our offices or you may fax the form back to the main office at 419-534-3780. Once the
form is received and approved you will be an active Online Banking member within 24 hours.
You will use your account number ONLY as your User ID and your password for the first time
entering will be your Six Digit Account Number AND the FIRST TWO initials of your LAST NAME
in CAPITAL LETTERS. You will then be asked to change your password at that time. Please write
down your information in a safe place, so you can retrieve your information easily. Remember
the Online Banking program is case sensitive, so if your password is in capital letters,
